The mission of the National Transition
Network (NTN) Transition Systems Change project
was
to strengthen the capacity of individual states
implementing five-year state systems change projects on
transition to effectively enhance school-to-work
policies, programs, services, and adult life outcomes for
youth with disabilities. NTN Transition Systems Change
goals included the following:
- Provide technical assistance and consultation in
essential areas of state project implementation.
- Generate high-quality, policy-relevant
information regarding ways in which the
experiences of individuals with disabilities and
families are positively influenced and shaped by
state-level efforts to improve transition
services, administrative practices.
- Disseminate policy and program-relevant
information to improve state and local policy and
program structures and achieve higher levels of
intergovernmental cooperation to benefit
individuals with disabilities and their families
as they transition from school to work and
community living.
Collaborators
The National Transition Network was housed at the
Institute on Community Integration (UAP), University of Minnesota. Dr.
David R. Johnson was the Director of NTN.
Dr. Ellen J. Emanuel served as Associate Director.
Collaborators of the NTN included the following

National Advisory Board
The NTN Advisory Board assisted in the development and
organization of NTN policies and activities, advised on
future school to work policy directions, served in a
consultative role to federal and state agencies serving
youth and adults with disabilities and as technical
consultants in planning evaluation and technical
assistance strategies, and participated in project
evaluation activities. Organizations represented on this
Board included the following:

The National Transition Network was funded through the
U.S. Department of Education, Office of Special Education
and Rehabilitative Services (Cooperative Agreement
#H168G20002).

National Center on Secondary Education and
Transition
The National Center on
Secondary Education and Transition (NCSET) at the
Institute on Community Integration coordinates national
resources, offers technical assistance, and
disseminates information related to secondary
education and transition for youth with disabilities in order to create
opportunities for youth to achieve successful futures.
All Means
All School-to-Work Project
The All Means All School-to-Work Project was a three year
project which recognized and celebrated the efforts of
local school-to-work partners who were committed to doing
"whatever it takes" to ensure access to and
benefit by all learners within their local school-to-work
system. It was a joint effort between the Institute on
Community Integration at the University of Minnesota and
the Minnesota Department of Children, Families, and

School-to-Work
Outreach Project
The School-to-Work Outreach Project at the Institute on
Community Integration identified exemplary school to work
models/practices/strategies that included students with
disabilities nationwide. Although this project has
completed it's identification process, the website
continues to disseminate information on exemplary
models/practices/strategies, as well as information on
school-to-work and other links available online.
